Practice Transformation

Payment reform and the rapid transition to telehealth is forcing medical practices to transform many aspects of how they deliver care.

Navigating Value-Based Payment

Quality Payment Program

The Medicare Access and CHIP Reauthorization Act of 2015 (MACRA) and the Quality Payment Program (QPP) transition from a fee-for-service payment system toward a system that rewards value. ACP provides expansive information, resources, and news on the subject.

Lessons from the Transforming Clinical Practice Initiative (TCPi)

The Transforming Clinical Practice Initiative (TCPi) was launched by CMS in 2015 to help clinicians get ready for value-based payment by providing free tools, resources and coaching to improve patient outcomes and reduce healthcare costs. ACP was a TCPi grantee.

These resources and inspirational videos were developed as part of the TCPi to help practices learn from others about quality improvement efforts to create a more effective, efficient and patient-centered healthcare system.
