TCPI Power Packs

The Transforming Clinical Practice Initiative (TCPi) was launched by CMS in 2015 to help clinicians get ready for value-based payment by providing free tools, resources and coaching to improve patient outcomes and reduce healthcare costs. ACP was a TCPi grantee.

TCPI Power Packs were developed by ACP and other grantees with input from CMS  to address specific challenges that a practice might face as it begins the process of clinical practice transformation.  Power Packs provide real life examples of practice transformation and actionable solutions to resolve healthcare challenges.

Six priority topics include:
