Registration Period for Consumer Assessment of Healthcare Providers and Systems (CAHPS) for MIPS Survey

If your group, subgroup, virtual group, or APM Entity (excluding Shared Savings Program ACOs) would like to administer the CAHPS for MIPS Survey for the 2026 performance period, you must register between April 1 and June 30, 2026. To report an MVP in the 2026 performance year, you’ll need to register between April 1 and November 30, 2026. During the registration process, you’ll also select if you want to submit as a subgroup. To report the CAHPS for MIPS Survey associated with an MVP, you must complete both the MVP registration and a separate CAHPS for MIPS Survey registration by 8 p.m. ET on June 30, 2026.

To register for the CAHPS for MIPS Survey:

  • Sign in to QPP.
  • Go to the Registration page.
  • Click “Start Registration.”
  • Click “Register and Edit CAHPS Registration.”
  • If you have multiple organizations on your account, select the organization you want to register and click “Register Group for CAHPS.”

MIPS Extreme and Uncontrollable Circumstances (EUC) Exception

MIPS eligible clinicians, groups, and virtual groups may submit an application to request reweighting of any or all MIPS performance categories if they've been affected by extreme and uncontrollable circumstances that impact these performance categories and aren’t covered by the automatic extreme and uncontrollable circumstances policy (see below). Clinicians that register to report a MIPS Value Pathway (MVP) as a subgroup will inherit any reweighting approved for their affiliated group. Alternative Payment Model (APM) Entities may apply but are required to request reweighting for all performance categories.

MIPS Promoting Interoperability (PI) Hardship Exception – Allows clinicians to request reweighting of the PI category due to issues like insufficient internet, CEHRT vendor problems, or extreme financial distress. Clinicians, groups, and virtual groups may submit a MIPS Promoting Interoperability Performance Category Hardship Exception application whether reporting traditional MIPS, an MVP, or the APP.

Both applications will be available spring 2026 and are due by 8pm ET on December 31, 2026.

2026 MIPS Annual Call for Quality and Cost Measures is Open

MIPS eligible clinicians, groups, and virtual groups may submit quality and cost measures for consideration. Currently, Measures for the quality performance category; and Measures for the cost performance category.

To report a quality and cost measure, you must register between March 19, 2026, to May 7, 2026. Measure developers can submit quality and cost measures for CMS’s consideration by completing the required fields and submitting applicable measure specifications and all supporting data files via the CMS Measures Under Consideration Entry/Review Information Tool (MERIT) by 8 p.m. ET on May 7, 2026. Candidate measures that are submitted via CMS MERIT will be considered for potential inclusion on the Measures Under Consideration (MUC) List, which is part of the pre-rulemaking process. The timeframe for candidate measures to be considered for inclusion in MIPS is a 2-year process. Only candidate quality and cost measures submitted by May 7, 2026, will be considered for inclusion in the MIPS quality or cost performance categories. Measures selected for inclusion on the MUC List would be considered for MIPS implementation in PY2028.
