Patient Centered Medical Home Video Project
The Patient Centered Medical Home (PCMH) video is the result of a collaborative project between leaders of the Georgia Chapter of the American College of Physicians and the Georgia Academy of Family Physicians. The video is a tool to help educate our physician members, along with our patients, business and industry leaders, and state and federal lawmakers on the PCMH model of care that aligns the highest quality of health care for patients with the greatest efficiency and cost effectiveness of the care provided especially by primary care physicians.
National leaders in primary care specialties, the American College of Physicians, the American Academy of Family Physicians, the American Academy of Pediatrics, and the American Osteopathic Association, representing over 330,000 primary care physicians developed the following joint principles to describe the key characteristics of the
Patient Centered Medical Home:
1. Personal physician
2. Physician directed medical practice
3. Whole person orientation
4. Care is coordinated and/or integrated
5. Quality and safety
6. Enhanced access
7. Appropriate payment
The PCMH model is only possible with Medicare payment reform that supports not only treatment of acute conditions and procedures, but also chronic disease management, prevention and health promotion. The Georgia Chapter of the ACP is actively engaged in advocating for the PCMH model to be implemented in our state and nation.
