The ACP Medical Home Builder (MHB) provides affordable, accessible on-line guidance and resources for practices involved in incremental quality improvement changes - or significant transformation of their practices. This online tool will help you:
- Quickly assess your practice in 7 key areas
- Identify specific tools & resources based on your needs
- Prepare for recognition as a Medical Home
Based on an intuitive interface, this self-paced program will guide practices through a thorough, yet simple, process (the ACP Practice Biopsy) for evaluating their practice in seven different important areas:
- Patient-Centered Care & Communication
- Access & Scheduling
- Organization of Practice
- Care Coordination & Transitions in Care
- Use of Technology
- Population Management
- Quality Improvement & Performance Improvement
A practice can focus on any module, in any order, without needing to complete the entire program. The MHB generates a color-coded print out of the biopsy results to help practices focus on functions that need attention, and also provides a crosswalk to the National Committee for Quality Assurance (NCQA) standards. The practice will be directed to tools (such as templates for office policies) and informative guides in a variety of formats (e.g., video, voice-over-slides, web-based, documents) based on responses to the Practice Biopsy questions. Users may choose to go directly to any resources, or use the search function to find material on a specific topic.
In addition to the practice assessment and dynamically linked resources, the MHB provides background information on the modules’ topics and uses a case study approach to highlight how to use the MHB. Each module follows a “practice” that completes the Practice Biopsy and implements changes using some of the resources provided. Click on the above icon to watch a silent 3 minute video tour.
