Comprehensive Primary Care Plus (CPC+)

Comprehensive Primary Care Plus (CPC+), CMS’s national advanced primary care medical home model, has added a second round of regions.

Applications are open now through July 13, 2017, for primary care practices in four new regions to apply.

CPC+ is a multi-payer program that began in January 2017, with approximately 3,000 primary care practices in the 14 Round 1 regions. CMS plans to add up to 1,000 practices from the four new regions in Round 2. Practices in CPC+ can participate in one of two tracks based on current practice capabilities. The initiative begins in Round 2 areas in January 2018 and runs for five years. CPC+ is an opportunity for primary care practices to receive additional payment outside of the fee-for-service system for providing comprehensive primary care. 

Why CPC+ Is Important

CPC+ is the only medical home model that is specifically identified in the Quality Payment Program/MACRA as an advanced Alternative Payment Model (APM). Practices that qualify for CPC+ will receive significant additional payment and, by automatically qualifying as an advanced APM, will be excluded from MIPS.

Who Can Apply

Any primary care practice in one of the four regions selected for Round 2 of CPC+ that is interested doing the work necessary to become a patient-centered medical home.  CPC+ will provide financial support to get the process underway.

CMS will be selecting up to 1,000 practices across the four regions participating in Round 2 of CPC+:

  • Louisiana (Statewide)
  • Nebraska (Statewide)
  • New York (Greater Buffalo Region)
  • North Dakota (Statewide)

Only practices in the four Round 2 regions above may apply. CMS is not accepting applications for practices in the 14 original Round 1 regions.
