Healthcare Coaching: ACP Advance Physician-Led Service

As a leading champion of Quality Improvement (QI) and High Value Care (HVC), ACP has collaborated with many practices, ACOs, and health systems to develop and implement small- to large-scale quality initiatives.

The ACP Advance healthcare coaching service provides peer-to-peer guidance, coaching from physician experts, and implementation support to help organizations achieve success in QI and HVC initiatives.

Working closely with each team, ACP Advance will engage physicians and clinical staff to help implement the initiatives that address the organization's unique challenges and help achieve their goals.

Physician participants from organizations that sign up for the ACP Advance coaching service are eligible for up to 50 AMA PRA Category 1 CreditsTM and ABIM MOC points. Physicians are also eligible to earn patient safety credit.

ACP partners with physician-led clinical teams to engage them in designing and advancing QI initiatives that are meaningful, improve efficiency, and reduce care costs in participating organizations. ACP will pair you with Expert Physician Coaches who are practicing internists who have already walked the QI and HVC path. They understand firsthand the challenges physicians face — from competing priorities and data collection to EHR hassles and meeting payer-dependent, wide-ranging quality measures. Expert Physician Coaches provide the tools, knowledge, and experience needed for each team to lead QI initiatives and achieve sustainable results.

Engaging the entire clinical team and administrative staff creates a unified, positive attitude toward quality performance across the organization and is central to the program's success. It helps drive a paradigm shift from physician-dependent care to high-functioning, team-based care, where everyone is invested in quality.

A unique benefit of ACP Advance is that participants will be part of a cohort of organizations that are also engaged in a QI initiative. This cohort model facilitates group learning, sharing best practices, and peer support.

ACP Advance puts physician-led clinical teams in the driver’s seat. Expert guidance and support from the coach ensures that participating clinicians will feel equipped to tackle change and improve quality.

ACP Advance provides support tailored to each organization. Expert coaches work with teams to conduct an in-depth practice assessment to identify gaps, understand why they exist, and define meaningful changes to achieve the greatest impact. Coaches then curate ACP resources designed to address specific gaps and goals.

Comprehensive Tools and Resources

  • Live, in-person QI training for 1 to 2 members of your clinical team in QI methodology based on ACP's curriculum

    • Individual coaching calls to identify gaps, set priorities and goals, and co-design a QI program that results in meaningful improvement for you and your patients
    • Group calls with all members of the cohort to cover relevant topics and provide the opportunity to share best practices with colleagues
    • Curated package of ACP resources to address gaps in QI, HVC, change management, joy in practice, and well-being

  1. "Current State" Assessment
    Representatives from each practice will complete the ACP Practice Assessment to help identify QI goals and priorities. Along with the coach, the representatives establish what's working, what’s not, what they want to change, and how to measure success.
  2. Comprehensive QI Training
    Up to two members from your organization will receive a full-day in-person training on QI methodology along with other members in the cohort. ACP’s online QI curriculum will be available to all members of your organization.
  3. Tailored QI Program
    With the team's input, the coach will help you develop a QI initiative that bridges aspirational goals with the practical steps needed to achieve them.
  4. Coach Guided Implementation
    Over the course of 12 months, each team will receive virtual coaching support from expert physicians via individual coaching calls, group webinars for sharing best practices, and educational sessions to maintain engagement level keep the program on track. 24/7 access to an online education platform is available for specialized learning and a member forum facilitates connection with peers from the cohort.

The American College of Physicians (ACP) is accredited by the Accreditation Council for Continuing Medical Education (ACCME) to provide continuing medical education for physicians.

The ACP designates this other activity (coaching and resources for practice improvement planning and implementation) for a maximum of 50 AMA PRA Category 1 Credit(s)TM. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn a maximum of 50 practice assessment MOC points and patient safety MOC credit in the American Board of Internal Medicine’s (ABIM) Maintenance of Certification (MOC) program. Participants will earn MOC points equivalent to the amount of CME credits claimed for the activity. It is the provider’s responsibility to submit participant completion information to ACCME for the purpose of granting ABIM MOC credit.
