Closing the Gap: Cardiovascular Risk and Secondary and Primary Prevention
The web-based program aims at providing practice teams with the tools necessary to implement quality improvement plans through practice measurement of patients with a current diagnosis of CAD, CVA or PAD, hypertension and chronic kidney disease. Quality and management indicators such as, blood pressure levels, use of anti-platelet therapy use, blood pressure lowering medications, behavior change counseling will be collected. Participants will receive instant real-time reports with information such as percent of patients on antiplatelet therapy and blood pressure therapies and blood pressure levels and the practices will use this data to inform their improvement strategies. This program offers up to 33 CME credits. Physicians also qualify for ABIM’s Part 4 MOC.
The American College of Physicians is accredited by the Accreditation Council for Continuing Medical Education (ACCME) to provide continuing medical education for physicians.
The American College of Physicians designates this educational activity for a maximum of 33 AMA PRA Category 1 credit(s)™. Physicians should only claim credit commensurate with the extent of their participation in the activity.
