How to Obtain a Plan’s Formulary
- The Plan’s website.
- The CMS formulary finder at www.Medicare.gov that will link you to the plan’s website. This is also available on ACP’s PIER clinical support tool.
- www.epocrates.com provides a free download of formularies in both web and PDA form, and includes cost sharing tier and substitution information for the formulary drugs. This provides the most comprehensive information.

- the Your Medicare and You 2007 Handbook,
- the local State Health Insurance Assistance Program (SHIP) or local Department of Aging. The phone numbers for these local agencies can be found at 1-800-677-1116 or online.
