How to Obtain a Plan’s Formulary

• The Plan’s website.
• The CMS formulary finder that will link you to the plan’s website. This is also available on ACP’s PIER clinical support tool.
• www.epocrates.com provides a free download of formularies in both web and PDA form, and includes cost sharing tier and substitution information for the formulary drugs. This provides the most comprehensive information.
