There are several resources to assist practices with transitioning to the Patient-Centered Medical Home model available from ACP, as well as from organizations with which ACP is partnering:
- ACP: Running a Practice – A central source of information on practice management, quality improvement, medical lab evaluation, health information technology, ethics and professionalism, and patient care and education.
- ACP: Diabetes Portal – Free tools, resources, and research supporting diabetes care for both patients and clinicians.
- ABMS: Improving Performance in Practice (www.abms.org) – Improving Performance in Practice (IPIP) is a state-based, nationally led quality improvement initiative funded in large part by the Robert Wood Johnson Foundation. Its origins are in collaboration among the primary care specialty societies to help physicians improve care in the office practice setting. ACP is a full partner in this initiative.
