Patients are curious about what a "Patient Centered Medical Home" is and this video was produced by Emmi Solutions in partnership with the Patient Centered Primary Care Collaborative (PCPCC) in an attempt to answer their questions. Many practices already provide the type of care outlined in the video, and it is by no means meant to be critical to those physicians who are doing their best to provide patient-centered care under the current structure.

Small Practice in America: Perspectives from Patients, Physicians and their Teams

Filmed over a period of three months in the spring of 2008, this video distills nearly 50 hours of interviews with clinicians in solo- and small-practices, staff members and patients. It was the final product of the Center for Practice Innovation project funded by the Physicians’ Foundation for Health Systems Excellence in January 2006. In this segment, Patient-Centered Care, patients share their thoughts about what they want and expect from their physicians and primary care office staff.
