Where are Demonstration Projects Happening?

Across the country, multistakeholder coalitions, including public and private payers, employers, governments, clinician organizations, academic institutions, consumer advocacy groups, and others, are working to develop and implement demonstration and pilot projects of the Patient-Centered Medical Home. While enthusiasm among these groups for the PCMH care model and its potential benefits is strong, the purpose of these demonstration projects is to test the model and measure in a credible way whether it works—particularly in terms of quality improvement and cost savings.

ACP has been actively involved in many of these discussions with the aim of ensuring they stay true to the Joint Principles (PDF) espoused by the primary care professional societies. To help facilitate consistency of PCMH demonstration projects with the Joint Principles, the primary care professional societies have released a set of 16 Guidelines for PCMH Demonstration Projects (PDF) intended to provide direction to projects in the planning phase and to facilitate more meaningful interpretation and understanding of the "lessons learned" from the different projects.
