Current Activity

Across the country, multi-stakeholder coalitions, including public and private payers, employers, clinician organizations, academic institutions, consumer advocacy groups, and others, are working to develop and test PCMHs projects. While enthusiasm among these groups for the PCMH care model and its potential benefits is strong, the purpose of these projects is to test the model and measure its effect, particularly in terms of quality improvement and cost savings.

ACP has been actively involved in many of these projects with the aim of ensuring the Joint Principles are espoused by the primary care professional societies. To help facilitate consistency of PCMH demonstration projects with the Joint Principles, the primary care professional societies have released a set of 16 Guidelines for PCMH Demonstration Projects intended to provide direction to projects in the planning phase and to facilitate more meaningful interpretation and understanding of the "lessons learned" from the different projects.
