“If it’s not documented, you didn’t do it.” Good documentation is critical, but in today’s environment it must be performed quickly, accurately, and efficiently. Using good forms and templates can save time, reduce medical errors, assure completeness, and minimize coding guesswork. The Practice Management Center’s (PMC) matrix of forms includes flow sheets, progress notes, problems lists, patient history questionnaires, plus registration, encounter, and telephone message forms.
Since no single form can meet the needs of every practice, these sample forms may also be adapted to fit your own practice setting. View PMC’s matrix of downloadable forms.

In addition to PMC resources, two outside commercial vendors sell pre-designed forms. They come “as is” but can be personalized with your name, address, etc., and may be available in Spanish. ACP does not endorse nor take responsibility for any outside vendor forms.
