Where are PCMH Demonstration Projects Happening?

Across the country, multistakeholder coalitions, including public and private payers, employers, governments, clinician organizations, academic institutions, consumer advocacy groups, and others, are working to develop and implement demonstration and pilot projects of the PCMH. While enthusiasm among these groups for the PCMH care model and its potential benefits is strong, the purpose of these demonstration projects is to test the model and measure in a credible way whether it works—particularly in terms of quality improvement and cost savings. ACP has been actively involved in many of these discussions with the aim of ensuring they stay true to the Joint Principles[PDF] espoused by the primary care professional societies. A subset of these projects—those that are the most developed and/or being implemented—are highlighted below:

Other areas where projects are under development with ACP involvement include Rhode Island, Colorado, Tennessee, Illinois, and Ohio. Many other stakeholders are also involved in these discussions, such as AAFP, AAP, AOA, the National Committee for Quality Assurance, quality improvement organizations, employers, business groups, academic and research institutions, public and private payers, consumers, and local and national coalitions (including the Patient-Centered Primary Care Collaborative).
