Resources for the PCMH
Practice Transformation
There are several resources to assist practices with transitioning to the patient-centered medical home model available from ACP, as well as from organizations with which ACP is partnering:
ACP: Running a Practice – A central source of information on practice management, quality improvement, medical lab evaluation, health information technology, ethics and professionalism, and patient care and education.
ACP: Diabetes Portal – Free tools, resources, and research supporting diabetes care for both patients and clinicians.
ABMS: Improving Performance in Practice (www.abms.org) – Improving Performance in Practice (IPIP) is a state-based, nationally led quality improvement initiative funded in large part by the Robert Wood Johnson Foundation. Its origins are in collaboration among the primary care specialty societies to help physicians improve care in the office practice setting. ACP is a full partner in this initiative.
In addition, ACP is in the process of developing new types of resources to better assist our members.
Purchaser Information
PCPCC: PCMH Purchaser Guide (www.pcpcc.net) - The Patient Centered Primary Care Collaborative (PCPCC), a coalition representing the country's national business leaders, consumer groups, organizations representing primary care physicians and other health care stakeholders, released its Purchaser Guide to the patient-centered medical home. The Purchaser Guide is a handbook for understanding the medical home model and taking action to advance its implementation. The guide provides supporting research, action steps, contracting language, and sample RFI language for employers/purchasers.
