ACP Foundation
The ACP Foundation, incorporated in 1999 and dissolved in 2012, supported the mission of ACP and strived to improve health communication by addressing the problem of low health literacy. The ACP Foundation developed patient-centered communications tools in a variety of formats that provide patients with important information to manage their health.
