ACP Foundation
The ACP Foundation, incorporated in 1999 and dissolved in 2012, supported the mission of ACP and strived to improve health communication by addressing the problem of low health literacy. The ACP Foundation developed patient-centered communications tools in a variety of formats that provide patients with important information to manage their health. To view or order any of the patient-centered tools, go to www.acponline.org/patient_tools.
The ACP Initiative on Atrial Fibrillation and Stroke Prevention is a unique opportunity for healthcare professionals, patients, and patient advocates to collaborate in creating meaningful change in the delivery of afib related care. For more information, go to the Afib website.
