Endometriosis with Ascites

Endometriosis is a common disease affecting 10% of premenopausal women. The incidence may be as high as 30-60% among infertile women. Although pelvic pain is the most common presenting symptom, other signs may indicate extrapelvic involvement. We present the case of a 20 year old patient with endometriosis presenting as massive ascites.
Endometriosis with ascites is unusual. There are about 30 cases reported in the literature. Initial presentation may raise suspicion of a malignancy. Medical management is effective. The case illustrates the importance of considering endometriosis in the differential diagnosis of ascites.
