Adult Immunization Outreach Program

The American College of Physicians, the Centers for Disease Control and Prevention, and the New Mexico Medical Review Association invite your practice to participate in an exciting immunization-focused quality improvement program called Inform, Implement, Immunize: ACP's Adult Immunization Outreach Program (I3).

I3 is a team-oriented, practice-based, educational intervention designed to help physicians and their staff develop strategies for immunizing their patients against vaccine preventable diseases such as influenza, HPV, herpes zoster, pneumococcal disease, pertussis, tetanus and Hepatitis B.

I3 teaches practices to measure their immunization rates and devise customized quality improvement plans for improving immunization services to their patients. This program incorporates the Chronic Care Model for systems change, the Institute of Healthcare Improvement (IHI) Breakthrough Series Model of social learning, and the Plan-Do-Study-Act (PDSA) cycle for quality improvement.

I3 will take place on Wednesday, July 9, 2008, at the Embassy Suites Hotel in Albuquerque, New Mexico.
