Vision Statement
Internal Medicine is a wonderful specialty, but the IM office-based practice is in trouble. Internists are undervalued, underpaid, and overworked. We were the intellectual leaders in medicine, but this is less often the case, as primary care internists are too busy with their volume-driven practices.
I envision a re-organization with the office-based internist as the head of a vertically integrated system, assisted by hospitalists and office-based midlevel providers. I have concerns about the rapid growth of hospitalists, physically and intellectually divorced from their office-based counterparts, often young and relatively inexperienced and the over-utilization of subspecialty consultations.
I envision IM subspecialists becoming more horizontally integrated with matching subspecialty surgeons, e.g. cardiology, cardio-thoracic surgery.
The ACP, with its national image and lobbying power must continue its leadership role, convincing political leaders and third party payors that internists are not the same as family practitioners or urgent care providers.
