Anyone with an interest in considering being a pilot practice for a PCMH project, please consider participating in this Webinar.
You and your office staff are invited to participate in a webinar to introduce our Kansas Patient Centered Medical Home (PCMH) Initiative webinar at 1 p.m. on Tuesday, February 8. Information can be found at http://www.kafponline.org/kspcmhinitiative.htm.
The webinar will provide a brief introduction to PCMH concepts, featuring Dr. Terry McGeeney, President and CEO of TransforMED. KAFP President Jen Brull, MD will discuss our Kansas PCMH Initiative, explaining the Pilot Project and how you can apply to participate. We have funding through our initiative to offer 8 practices (with 5 FTE physicians or fewer) the opportunity to work with TransforMED to become PCMHs.
